The Smartlipo is a laser system that can reach extremely high frequencies for very brief periods, even several millionths of a second, using electronic control for the emission of power and laser pulses.

Smartlipo is considered to be a safe, minimally invasive laser liposuction surgical procedure using an advanced laser to reduce unwanted stubborn fat from most areas of the body. The procedure is carried out under local anaesthetic, light IV sedation or general anaesthetic, eliminating the need for aggressive surgery and without leaving any marks.

Dr Ayham Al-Ayoubi’s view on Smartlipo treatment
‘The first time I came across Smartlipo was when I read an article in 2002 by Dr Alberto Goldman, a plastic surgeon in Brazil; it was entitled Smartlipo/Laser lipolysis. This treatment fascinated me as an ENT, Facial Plastic and Laser Surgeon.
I tried to bring Smartlipo into the UK but at that time there wasn’t enough clinical studies available for it to be approved in this country. Approval was finally given and I was the first surgeon to introduce the new laser technique from Italy into the UK in 2006.
Speaking specifically about Smartlipo/Laser lipolysis, fat reduction and body sculpting, we have to understand the anatomy and physiology of the fat structure in the human body, where we have deep loose fat and superficial fat which sits just beneath the skin. This superficial fat is extremely vascular and the fat cells are positioned very closely together.
Prior to launching Smartlipo into the UK, we were using traditional liposuction to achieve fat reduction. Traditional liposuction is performed by making an incision into the skin to suction out the deep fat in a safe way, with very good results in the majority of cases. However, it is not possible to suck out the superficial fat because of the high risk of bleeding and the damage that could happen to the skin from the inside when we remove that fat.
With some patients (approximately 20 per cent of cases) it can result in a “collapsed banana skin effect” whereby the skin isn’t supported any longer.
As surgeons, another problem we have found with traditional liposuction is that we are restricted in the areas that we can perform the procedure i.e… under arm liposuction surgery could result in numbness in the hands if the nerves have been damaged by the powerful suction. Also, under the chin, which is an extremely vascular area, the bleeding could be severe during surgery.
The principle is that instead of only sucking the fat out, during Smartlipo treatment, the laser targets the fat cell membrane and punctures it. Like a balloon filled with water, we pierce the fat cells and the fat leaks out and gets absorbed and drained through the lymphatic system.
I started doing Smartlipo procedures with the first generation Smartlipo machine (Deka System) which had great results in small areas, particularly in terms of skin tightening. The skin tightening effect is due to the heat of the laser increasing up to 38 – 40o and it produces an inflammation which tightens the skin.
I must stress here that Smartlipo is not a lunch break fat removal treatment. Proper techniques are required to perform the treatment.
Smartlipo – Laser-guided liposuction is not just placing a cannula under the skin for a short time and then saying: “we did the procedure!” Physicians can spend a whole day just learning how to enhance the results of liposuction. It is an exacting and time-consuming process that requires attention to detail and consistency throughout the operating area.
Most importantly, it is the delivery of a significant amount of energy to the deep layer and a complete heating of the dermal layer to a temperature at or close to 40oC. A typical lower abdomen would require between 50 and 80 thousand joules of energy to achieve the desired results of fat reduction.

SmartLipo Triplex – since 2010
A new member to the laser lipolysis field, the SmartLipo Triplex, offers greater fat reduction and skin tightening. The SmartLipo Triplex Laser has developed the SmartLipo MPX series even further.
The Triplex adds another benefit to the original Smartlipo system with an additional wavelength. It is the first laser lipolysis technology that uses three wavelengths which are well absorbed by fat.
SmartLipo Triplex adds a 1440 nm wavelength to the MPX generation of the laser. The three wavelengths are combined in one laser output. The wavelengths fire sequentially, liquefying the fat and tightening the skin with Sculptra.
The 1064 and 1440 wavelengths combine for fat liquefaction and the 1064 and 1320 wavelengths combine for Sculptra and blood vessel coagulation.
This combination provides both fat reduction and skin tightening, therefore patients can often avoid additional procedures to remove the loose skin people often experience after liposuction.

Dr Ayham Al-Ayoubi of London Medical and Aesthetic Clinic led the way yet again in 2010 by introducing the very first evolutionary SmartLipo Triplex from the USA into the UK and Europe.
Who can benefit from Smartlipo?
Smartlipo – laser lipolysis is safe for anyone of any age. Positive results depend more on good skin tone and level of physical activity than biological age.
The ideal person for Smartlipo-Laserlipolysis is someone with generally good health, who carries out regular exercise, has good eating habits and whose weight does not exceed 25% – 30% more than their ideal body weight.
Some medical conditions may limit the use of this treatment option. Those with heart, kidney and liver disorders or on anticoagulants are not recommended to be treated with Smartlipo – laser lipolysis.
Patients who want Smartlipo but be unsuitable
Over the past few years there has been on average a 15% annual growth in body shaping treatments. Patients are more aware of the increasing number of non-invasive, semi-invasive and combination procedures.
The Smartlipo system has become the fastest growing treatment for body sculpting in the world. However, not every patient is suitable for Smartlipo treatment .i.e. those who are overweight and do not have a healthy life style or do not exercise enough or patients who have very large areas of fat.
Those patients are usually referred to dieticians or nutritionists who can help them lose weight and improve their life style and dieting habits. Also, in some cases alternative treatments are recommended to the patients such as traditional liposuction or surgical techniques for fat reduction.
Equally important are patients who do not have realistic expectations of the end results. Therefore patient selection is extremely important during the initial consultation. We strongly believe that the consultation must be carried out by the surgeon himself who will perform the Smartlipo treatment.
To help the patient to envisage the end results with Smartlipo, Dr Ayham Al-Ayoubi has developed a simple chart where he measures each area to be treated in cm and then measure the areas again in each follow up visit to see how much reduction has been made.
Any technique, whether it is surgery, traditional liposuction, or Smartlipo, will only have the optimum outcome if the patient is committed to being fit and healthy to maintain the results. Realistic expectations of the final result will usually take up to three or four months post-treatment.

What can be expected?
Even though Smartlipo – laser lipolysis by itself is capable of remodelling extensive areas with excess fat, it is not capable of altering the basic structures of the skeleton and muscles.
Nearly all the areas of the body can be improved with Smatrlipo Laser lipolysis. During the initial consultation, we will describe the results you can expect, and the order in which these areas should be treated. In this way, we can discuss a treatment plan to achieve the best body shape to suit you.
How effective is the treatment?
Visible results are seen immediately with suction 9 Smartlipo Laser Assisted Liposuction . However, it will take 1-2 months post-treatment to start seeing significant results of the treatment. If large amounts of fat are to be removed, a stepped programme will probably be advised to allow for gradual reshaping. Fat can be removed from almost any part of the body.
Side effects of Smartlipo
Smartlipo – laser assisted liposuction – LAL is minimally invasive surgical laser procedure to remove excess fat, and to tighten the skin to improve the look of the treated areas. It is not a weight reducing technique. It will not give you the look on the treated areas that you had when you were younger, or before you had children.
The operation is usually performed under a local anaesthetic, IV sedation or genral anaesthia. The duration of the procedure is 1-3 hours.
Dr Ayham Al-Ayoubi explains that Smartlipo is a laser surgical procedure and like any other laser surgical treatments it could have some side effects and complications and it’s the duty of the surgeon to minimize the side effects, possible risks and complications of the procedure; even so they cannot be eliminated.
The following lists of side effects and complications has been reported (even so they are rare) but the patients has to be aware of them.
During the Smartlipo consultation with Dr Ayham Al-Ayoubi, patients will have the chance to have a full discussion about the possible risks or side effects in more details.
- Haematoma – Small haematomas are usually reabsorbed. A large one very rarely may need further surgery.
- Seroma – A collection of fluid in the treated areas (extremely rare to happen) which may need aspiration.
- Infection – Bacterial or fungal infection may occur, which may require antibiotics. It may lead to abscess information. (very rare)
- Scarring – a very small 1-3 mm scar will be made along the treated areas – those could be a permanent mark, but will improve in time. Sometimes the scars go.
- Diminished or altered sensation – the nerves to the skin are interrupted during procedure; you may have numbness or less feeling over the treated areas. The sensation returns over several months. Some diminished sensation may last indefinitely.
- Swelling – may last few days to month, but it may take up to 2 months to improve.
- Pigmentation – can occur either hypo pigmentation or hyper pigmentation. Avoid sun exposure, including sun beds, for at least 2 months.
- Bruising and discolouration will last for a few days/weeks.
- Pain and discomfort – will be experienced. The amount varies with the individual’s tolerance. Take analgesia as required.
- Poor healing – can occur in some areas.
- Asymmetry – pre-operative asymmetry cannot be guaranteed to be corrected during procedure.
- Excessive tightness and bloating – is common after surgery and is usually temporary.
- Temporary lumps under the skin – these will resolve in time.
Life threatening complications are rare – they may include: Deep vein thrombosis, pulmonary embolism, and respiratory decompensation.
Smartlipo is a minimally invasive surgical procedure and is administered under a local anaesthetic, iv sedation or genral anaesthia.
Smartlip is a very effective treatment. However, Smartlipo like any weight loss programme needs a sensible diet and a healthy lifestyle to maintain peak condition and efficacy.
Smartlipo laser lipolysis is not designed to treat obesity. However, Smartlipo with suction can be used to treat two or more large areas in a person who is overweight and who finds it quite difficult to lose weight. A person with obesity, who requests laser lipolysis, is always recommended to try losing weight by other means first.
Smartlipo can be used for skin tightening alone, especially with the Smartlipo MPX system which has 2 laser wavelengths. The setting of the laser can be used in such a way to treat loose skin with very effective results.
Smartlipo (treatment) and specifically Smartlipo MPX system has the ability not only to reduce fat and tighten the skin but has the ability to treat cellulite. The laser energy can be set up in such a way that it is designed to tighten the skin and improve cellulite appearance by giving significant skin tightening effects. As regards to stretch mark improvement, there is so far no proven clinical study. However there are some patients who have noticed some improvement regarding their stretch marks after Smartlipo treatment.
After the Smartlipo treatment the patient usually has a significant skin tightening effect especially if the surgeon uses the Smartlipo MPX system or Smartlipo Triplex. These systems have the ability to introduce an adequate amount of laser energy, heating the dermis (skin) and the fat within safe parameters, which will give the skin tightening effect.
How does Smartlipo work?
Smartlipo’s pulsating laser beam has a photo-mechanical effect rupturing the fat cells’ membrane. Also it has a thermal effect which melts the fractured fat cells and coagulates small blood vessels which reduce the bleeding to minimum. Both effects together (Photo-dynamic and Thermal) will give the skin tightening results.
The areas to be treated are marked with a special pencil and each is divided into segments of 5x5cm. A small incision 1-2mm is made with a micro-scalpel in which the local anaesthetic is injected. The local anaesthesia is carried out via longitudinal infiltration of the subcutaneous fatty tissue in anything from ten to twenty directions in order to form a network.
1- Melting the loose fat
The optical fibre of the laser is inserted into the same incision created for the infiltration of the anaesthetic solution. Its transit through the skin is monitored by means of the guiding light on the laser device itself. The actual treatment begins by slowly moving forwards lengthwise following the lines traced on the skin and trying to constantly maintain the same speed of movement.
2- Lasering the superficial fat and tightening the skin at the same time
Over the last two years Dr Ayham Al-Ayoubi has taken the Smartlipo treatment further and developed his own technique for patients with much larger areas of fat. This combines Smartlipo and gentle suction at the same time.
The deep fat is lasered first followed by the superficial fat and finally the fat is suctioned out. This produces great results and is relatively painless. Dr Ayham Al-Ayoubi always reminds his patients that it is not possible to have any procedure without some discomfort, but this is a very good and safe procedure.
The skin tightening in some areas like hips is immediate, patients often like to see results straightaway, they don’t like to be told that it can take many months. It is also done under local anaesthetic so there is no need for a general anaesthetic or to stay overnight which also means that it can be done on patients who aren’t suitable for a general. There are no stitches, just a very small incisions that heal very well and quickly. By three or so months the patients can see the final result.
Does it require admission to hospital?
No, the laser lipolysis treatment does not require hospitalisation or even a general anaesthetic as it is carried out in the clinic. The post-treatment marks like haematomas and inflammation – common to other methods of surgical body modelling – are considerably reduced.
Method
- Treatment is carried out in a sterile environment.
- The areas to be treated are marked with a special pencil, and lines are traced outwards from one single point in which the local anaesthetic is injected.
- The local anaesthesia is carried out with a peristaltic B&S bottle via longitudinal infiltration of the subcutaneous fatty tissue in anything from ten to twenty directions in order to form a network.
- The optical fibre of the laser is inserted into the same incision created for the infiltration of the anaesthetic solution.
- In order to facilitate the insertion of the cannula, a small incision must be made with a micro-scalpel.
- The laser is then turned on and the optical fibre is inserted into the adiposity. Its transit through the skin is monitored by means of the guiding light on the laser device itself.
- The actual treatment begins by slowly moving forwards lengthwise following the lines traced on the skin and trying to constantly maintain the same speed of movement.
Results
At the end of the treatment the fatty tissue will be more compact and have greater tone without any surface irregularities. Due to the lipolysis effect produced by the laser, the body contours improve day by day, providing the results within 1 to 2 months post-treatment. However, it usually takes between 3 to 4 months to see the final results, which vary individually between clients.
Laser lipolysis allows for recovery and the patient can return to a normal lifestyle after treatment.
How many treatments will I need?
This very much depends on the amount of fat to be removed from each area. More often than not one treatment will achieve the desired effect. The recommended amount of fat removed from one area is approximately 250/300cc.
How long does a treatment take?
Treatment takes 1-2 hours depending on the number of areas. Most patients can return to work the following day after treatment.
Could I have an even result with Smartlipo treatment after fat reduction?
NO, it is actually the Smartlipo treatment that can be used to treat asymmetrical shaping, as the surgeon can apply differing amounts of varying laser energy to different areas so he can sculpture the body shape with the laser.

1- The laser beam, inserted into the hypoderma by means of the cannula, affects the adipocytes and surrounding tissues.
2- – Thanks to the photo-mechanical effect, part of the laser energy ruptures the membrane of the adipocytes closest to the end of the fibre.
3- In some cases the adipocytes are not destroyed immediately. Nevertheless, the laser energy still manages to alter the permeability of their membrane causing them to swell which leads to their destruction within a month or two.

Dr Ayham Al-Ayoubi has been using the Smartlipo since 2006 and invented and devolped the conset of Laser Assisted Liposuction in 2007.
In 2008, he introduced the Smartlipo MPX, the second new generation of Smartlipo systems from the USA into the UK and Europe. Like its predecessors, the Smartlipo MPX enhances the breakdown of fat, reduces bruising through the coagulation of blood vessels and enhances the skin tightening effect by stimulating fibroblasts and collagen production. The difference is that the speed and efficacy are enhanced with the new Smartlipo MPX system.
With its entirely different platform, Smartlipo MPX features 20w for the 1064 nm wavelength and 12w for the 1320 nm wavelength. The 1064 nm wavelength concentrates on the blood vessels and fat cells and this laser beam fired under the skin improves skin tone and acts on collagen fibres, all without causing much damage. Whereas the 1320 nm wavelength has a higher affinity for fat and this laser energy melts the fat by bursting the cells, making the fat easier to suck out.
This procedure increases the efficiency of not only the heating, but the entire process. This means we can destroy more fat cells in a shorter period of time, so not only is the patient having a much faster treatment but we can do much bigger areas and multiple areas within the same treatment.
The two lasers work both at the same time but we can change the settings which gives us a greater degree of flexibility in what we can offer to the patients.
Smartlipo MPX also utilizes SmartSense safety features which regulate the level of laser energy being delivered in relation to the speed at which the headpiece is being moved, thus providing constant and uniform energy delivery throughout the treatment. SmartSense prevents laser discharges if the headpiece stops moving but resumes when it starts to move again.
In terms of practice positioning, Smartlipo has been used as a stand-alone treatment as well as an adjunctive procedure to enhance the results of traditional liposuction.
Major surgical techniques for fat reduction leave a noticeable scar, involves hospitalization and requires at least ten days of downtime.
Traditional liposuction is an imprecise tool because it is limited to the deep fatty layers around the muscle.

The surgical technique to remove fat has been performed for many years – the most common example being the tummy tuck – and it works in a very simple way.
You cut the skin, remove the fat and then re-drape the skin.
There are some advantages: you can remove a lot of fat, it is good for the surgeon because they can see what they are doing and you can achieve skin tightening at the same time by removing any loose skin.
The downsides though often outweigh the advantages: it is performed under general anaesthetic, entails a lengthy period of downtime and you are left with a very obvious scar.
Traditional liposuction, early 1980s
Traditional liposuction was introduced in the early 1980s and became the standard technique for the next twenty years,
It works by making a small incision in the skin and sucking out the fat with the liposuction device, but the problem lies in the fact that we have two different types of fat: deep, loose fat and superficial fat.
Liposuction tackles the deep fat but can’t remove the superficial fat because it is very vascular. If you attempt to remove it with liposuction you risk too much bleeding and damage to the skin.
It was obvious; we needed to find a solution to the superficial fat.
Other alternatives, the 1990s
All research at the time was dedicated to finding ways to tackle the superficial fat. The first approach was applying heat to the surface of the skin in an attempt to emulsify the superficial fat underneath – not through an incision – and lots of different forms of energy were experimented with, including laser, radiofrequency and ultrasound.
These techniques were often combined with traditional liposuction but they didn’t have the desired result because the more energy you apply through the dermis, the more you risk damaging or burning the skin before you even have an impact on the fat underneath.
Smartlipo since year 2000
Finally, we had a breakthrough; Smartlipo works by making a tiny incision, under local anaesthetic, and then emulsifying the fat by rupturing the membrane of the fat cell using the light, not the heat, of the laser. The contents of the fat cell leak out and the cell itself no longer has a function so is removed by the body.
The first machine had six watt energy and was fantastic in small areas such as under the chin, and for small amounts of tummy or thigh fat but I had many patients wanting much larger areas dealt with. The machine wasn’t producing the results wanted and the length of the procedures meant discomfort for the patients.
Smartlipo MPX, since year 2008
The company was introducing more and more powerful machines which were producing better results and more comfortable patient experiences, then their sister company in the US, Cynosure, developed Smartlipo MPX.
MPX stands for multiple lasers and the machine employs two lasers. One targets fat membrane and the other targets blood vessels.
The current Smartlipo MPX is also so much more powerful than the original machine from six watts to 46 watts and that increase in power means you go through tissue easier and tackle larger areas in less time. It also means increased skin tightening which is crucial when you’re tackling larger areas of fat.
Smartlipo Triplex, since year 2010
The SmartLipo Triplex, offers greater fat reduction and skin tightening.The SmartLipo Triplex Laser is an improvement upon the SmartLipo MPX series.The Triplex adds another benefit to the original Smartlipo system with an additional wavelength. Bodytite. Introduced into the UK in 2009 by Dr Ayham Al-Ayoubi. This uses radiofrequency and, at 75 watts, is very powerful. It is suitable for large areas but doesn’t produce the same skin tightening as Smartlipo MPX or Smartlipo Triplex , nor does it have the flexibility when dealing with rounded areas such as the inner thigh. It doesn’t have the same range of safety features as Smartlipo MPX, which is invaluable even for an experienced surgeon.
Vaser. Reliant on ultrasound, this is good for small areas but doesn’t give the skin tightening of Smartlipo. It works by vibrating the fat cells and damaging them but it can also damage the tissue around it causing pain and bruising. It can also cause irregularities. It has its place but has pros and cons as all systems do.
Other lasers. Targeting the fat cells not the membrane, they can produce unwanted dead tissue under the skin.

Brief Information about Smartlipo Laser Body Sculpting
The success we experienced with the original Smartlipo device which was invented by DEKA Laser Company in year 2000 after 12 years of extensive studies, was greater than we thought it would be.
Cosmetic and plastic surgeons recognise patient’s demands for less invasive cosmetic procedures. It has been realised by everyone that having a clinic based only on plastic surgery is not enough, as these patients are going to clinics that offer Smartlipo treatments because Smartlipo provides good results and is less invasive than previous more invasive treatments.
Smartlipo is not a lunch break fat removal treatment and Dr Ayham Al-Ayoubi stresses this. Experienced techniques are required.
Smartlipo, also Laser-guided liposuction is not just about placing a cannula under the skin for a short amount of time and saying: “The procedure is done!” Surgeons and physicians can spend a whole day alone just learning how to enhance the results of liposuction. It is a time-consuming process that requires attention to detail and importantly consistency throughout the whole procedure.

Who is suitable for Smartlipo
Laserlipolysis is popular for people aged 18 to 60 years as it is suitable for any age. Many have benefited from laserlipolysis. Positive results are not affected by age but depend more on good skin tone and level of physical activity.
The ideal candidate for laserlipolysis is generally in good health, who carries out regular exercise, as well as having good eating habits and whose excess weight does not exceed 25% more than their ideal body weight.
Those with heart, kidney and liver disorders or on anticoagulants are not recommended to be treated with laserlipolysis. These medical conditions may limit the use of this treatment option.
Patients who want Smartlipo but may be unsuitable
Over the past seven years since Smartlipo was introduced, there has been on average a 15% annual growth in body shaping treatments. Patients are more aware of the options for non-invasive, semi-invasive and combination procedures due to the media and internet.
The Smartlipo system has become the fastest growing treatment for body sculpting in the world. However, not every patient is suitable for Smartlipo treatment .i.e. those who are overweight and do not have a healthy life style or do not exercise enough or patients who have very large areas of fat.
To overcome this or to prepare unsuitable patients for the Smartlipo treatment, those patients are usually referred to dieticians or nutritionists who can help them lose weight and improve their life style and dieting habits. Also, in some cases alternative treatments are recommended to the patients such as traditional liposuction or surgical techniques for fat reduction.
Equally important patients must have a realistic expectation of the results from the Smartlipo treatment; therefore the initial consultation is very important for patient selection. We strongly believe that the consultation should be carried out by the physician himself who will perform the Smartlipo treatment.
During the consultation, and each follow up visit, to help the patient to envisage the end results initially and then to see how much reduction has been achieved with Smartlipo, Dr Ayham Ayoubi has developed a simple but important chart where he measures each area to be treated in cm and then measure the areas again, allowing the patient to follow the results they have from the Smartlipo or laserlipolysis treatment.
The patient being fit and healthy to maintain the results and have the optimum outcome is important with any technique, whether it is surgery, traditional liposuction, or Smartlipo. Realistic expectations of the final result will usually take up to three or four months post-treatment.
Where can Smartlipo be used?
Thanks to the advanced Smartlipo, the cannula is very small, allowing the Smartlipo to be performed on the stomach, love handles, legs, thighs, saddle bags, neck, arms, calves and other trouble areas.

Smartlipo step by step
Smartlipo has a photo-mechanical laser beam effect which ruptures the fat cells’ membrane. Also it has thermal effect which melts the fractured fat cells and coagulates small blood vessels which reduces the bleeding to minimum. Working together (Photo-dynamic and Thermal) will give the skin tightening results.
When we know the areas to be treated, they are marked with a special pencil and each is divided into segments of 5x5cm. A small incision 1-2mm is made with a micro-scalpel in which the local anaesthetic is injected. The local anaesthesia is carried out via longitudinal infiltration of the subcutaneous fatty tissue in anything from ten to twenty directions in order to form a network.
From the same incision that is made for the infiltration of the anaesthetic solution the optical fibre of the laser is inserted with its transit through the skin monitored by the light on the laser itself. The actual treatment begins by slowly moving forwards lengthwise following the lines traced on the skin and trying to constantly maintain the same speed of movement.
Over the last two years Dr Aham Al-Ayoubi has taken the Smartlipo treatment further and developed his own technique for patients with much larger areas of fat. This combines Smartlipo and gentle suction at the same time and works by having the deep fat lasered first followed by the superficial fat and finally the fat is suctioned out. Dr Ayham Al-Ayoubi always reminds his patients that it is not possible to have any procedure without some discomfort, but this is a very good and safe procedure that is relatively painless.
Some areas like hips show immediate results, patients often like to see results straightaway, they don’t like to be told that it can take many months. It is also done under local anaesthetic or a light IV sedation so there is no need for a general anaesthetic or to stay overnight which also means that it is suitable for patients that cannot go under general. There are no stitches, just a very small incision that heals very well and quickly. The patient will see the results in 3-4 months.
Method of treatment
The treatment is carried out in a sterile environment and the areas to be treated are marked with a special pencil, and lines are traced outwards from a single point with the local anaesthetic is injected. The local anaesthesia is carried out with a peristaltic B&S bottle via longitudinal infiltration of the subcutaneous fatty tissue in anything from ten to twenty directions in order to form a network into the same incision created for the infiltration of the anaesthetic solution the optical fibre of the laser is inserted. A small incision is then made to facilitate the insertion of the cannula. The actual treatment begins by slowly moving forwards lengthwise following the lines traced on the skin and trying to constantly maintain the same speed of movement.
Results
At the end of the treatment the fatty tissue will be more compact and have greater tone without any surface irregularities. Due to the lipolysis effect produced by the laser, the body contours improve day by day, providing the results within 1 to 2 months post-treatment.
How many treatments will I need?
This very much depends on the amount of fat to be removed from each area. More often than not one treatment will achieve the desired effect.
How long does a treatment take?
1-2 hours depending on the number of areas.
Could I have an even result with Smartlipo treatment after fat reduction?
No, it is actually the Smartlipo treatment that can be used to treat asymmetrical shaping, as the surgeon can apply differing amounts of varying laser energy to different areas so he can sculpture the body shape with the laser.

Nearly all the areas of the body can be improved with laserlipolysis. You have the option of treating one or more areas at the same time, but even though laserplipolysis by itself is capable of remodelling extensive areas with excess fat, it is not capable of altering the basic structures of the skeleton and muscles.

How effective is the treatment?
Fat can be removed from almost any part of the body and visible results are seen immediately with suction. However, it can take up to 2-3 months post-treatment to start seeing significant results of the treatment.
In some instances a stepped programme will probably be advised to allow for gradual reshaping if large amounts of fat are to be removed.
Are the results permanent?
Yes, however, with Smartlipo and any weight loss programme a sensible diet and a healthy lifestyle is needed to be maintained at peak condition and efficacy making it permanent and a very effective treatment.
Can laserlipolysis be used to treat obesity?
No, people with obesity, who request laserlipolysis, are always recommended to try losing weight by other means first as laserlipolysis is not designed to treat obesity. However, Smartlipo with suction can be used to treat two or more large areas in a person who is overweight and who finds it quite difficult to lose weight, making it that first step.
Can Smartlipo be used for skin tightening alone?
Yes, especially with the Smartlipo MPX system this has 2 laser wavelengths. The setting of the laser can be used in such a way to treat loose skin with very effective results.
Can Smartlipo work on cellulite and stretch marks?
Smartlipo (treatment) and specifically Smartlipo MPX system has the ability not only to reduce fat and tighten the skin but has the ability to treat cellulite. The laser energy can be set up in such a way that it is designed to tighten the skin and improve cellulite appearance by giving significant skin tightening effects. As regards to stretch mark improvement, there is so far no proven clinical study. However there are some patients who have noticed some improvement regarding their stretch marks after Smartlipo treatment.
Will I have saggy skin after the Smartlipo treatment?
No, with the Smartlipo treatment the patient usually has significant skin tightening especially if the surgeon uses the Smartlipo MPX system. This system has the ability to introduce enough laser energy, heating the skin (dermis) and the fat within safe parameters, which will give the skin a tightening effect.
Smartlipo vs. other laser techniques
Since introducing the second new generation of Smartlipo systems from the USA into the UK and Europe, Dr Ayham Al-Ayoubi has been using the Smartlipo MPX since September 2008 when like its predecessors, the Smartlipo MPX also enhances the breakdown of fat, reduces bruising through the coagulation of blood vessels and enhances the skin tightening effect by stimulating fibroblasts and collagen production. The difference between the Smartlipo MPX and previous systems is that the speed and efficiency are enhanced with the new Smartlipo MPX system.
The 1064 nm wave length concentrates on blood vessels and fat cells and this laser beam fires under the skin and improves the skin tone and acts on the collagen fibres, and it does all without causing much damage. This allows the procedure to increase the efficiency of the heating and the entire process. Meaning fat cells are destroyed in a shorter period of time, so the patient is able to leave the clinic much sooner and we can do larger and multiple areas within the same time
Both lasers work at the same time but the settings can be changed which gives us a greater degree of flexibility so we can offer this to the patients.
Smartlipo MPX has Smart Sense safety features which are able to regulate the level of laser energy being administered in relation to the speed at which the hand piece is being moved, so this is providing constant and uniform energy delivery throughout the entire treatment. Smart sense safety features prevents laser discharges if the hand piece was to stop moving but restarts when it starts to remove.
Smartlipo can be used as a stand-alone treatment as well as an procedure to enhance the results of traditional liposuction.
Traditional liposuction can also supplement Smartlipo, it does this by melting the fat, then the fat stays behind and then you remove the fat with small cannulas. Dr Al-Ayoubi has noticed that patients that will not consider liposuction because, it involves hospitalization, a minimum of at least seven days of downtime and can leave saggy skin find that they prefer to opt for Smartlipo as there is no hospitalisation and no downtime. Another important factor why patients may prefer to go for the Smartlipo MPX instead of traditional liposuction is the scar reduction that is with Smartlipo than with traditional liposuction
A patient with loose saddle bags (thighs) had a super result by Dr Al-Ayoubi when he used the Smartlipo MPX. The patient would be left with thin floppy thighs after traditional liposuction. When she was treated with the Smartlipo MPX, the result was slim and non-floppy thighs.
Traditional liposuction is not a precise tool as it is limited to the deep fat layers around the muscle. Smartlipo we can treat the superficial fat layers and get underneath the dermis with the small (2 mm) cannula.
When this is done the skin will shrink because we release it from the fatty layer. Then when we heat the skin to about 38-40oC, we can achieve additional shrinkage from reticular Sculptra and reformation. This can be done more quickly and precisely with Smartlipo MPX’s two wavelengths unlike the previous original Smartlipo or ultrasound, the surgery would be very time consuming.
Smartlipo has become very popular in the UK because downtime is reduced and results are of a high quality. Dr Ayham Al-Ayoubi is expecting ongoing improved results and to treat difficult cases such as both large and small areas, with the Smartlipo MPX. Skin tightening result is immediate with Smartlipo MPX. Patients like to see results straight away and do not like to be told that it can take a few weeks to months. It is done under local anaesthetic or under a light IV sedation meaning that there is no need for a general anaesthetic or hospitalization. There are no stitches just a very small incision that heals very well, meaning no scarring.
It is important to remember that even though we can take out a litre and maybe more of fat this procedure is all about body reshaping and sculpting, not so much about weight loss.

Side effects of smartlipo
No, occasionally, however, there may be a temporary bruising of the skin around the area of treatment and slight discomfort usually lasting for a day or two.
Does it hurt?

I once tried to introduce Smartlipo into the UK but in 2002 there were not enough clinical studies available for it to be approved in this country at the time. When approval was finally given I was the first Surgeon to introduce the new Smartlipo/Laser Lipolysis laser from Italy into the UK in 2006.
Speaking only about Smartlipo/Laser Lipolysis, fat reduction and body sculpting, we have to understand the anatomy and physiology of the fat structure in the human body, where we have deep loose fat and superficial fat which sits just beneath the skin. This superficial fat is extremely vascular and the fat cells are positioned very closely together.
Before introducing the Smartlipo into the UK, surgeons were using traditional liposuction to achieve fat reduction. Traditional liposuction is performed by making an incision to the skin to suck out the deep fat in a safe way, with very good results. Even so it is not possible to suck out any of the superficial fat because of the high risk of bleeding and the damage that could be caused to the skin from the inside when we remove that fat but now that has changed since the Smartlipo we can now safely suck out the superficial fat.
With about 20 per cent of cases of our patients traditional liposuction surgery can result in a collapsed banana skin effect where the skin is not able to be supported any longer.
As Surgeons, another problem we found with traditional liposuction is that we are restricted in the areas that we can perform the procedure i.e. liposuction for under the arm (bingo wings) surgery could result in numbness in the hands from where the nerves have been damaged by the powerful suction. Even under the chin, where it is an extremely vascular area, the bleeding could be severe during surgery so we tackled all of these problems by introducing the Smartlipo/Laser Lipolysis allowing us to target all areas including underarms and under the chin.
The principle is that instead of removing the fat with suction, during Smartlipo treatment, the laser will target the fat cells and then puncture them similar to water filled balloon, the fat cells are pierced and then the fat leaks out and gets absorbed and drained through the lymphatic system.
I started doing Smartlipo procedures with the first generation Smartlipo machine produced by the Deka System and it had great results in small areas, especially in terms of the skin tightening. The skin tightening effect is achieved because the heat of the laser goes up to 38-40oC and it produces an inflammation which tightens the skin.

The evolutionary thrid generation SmartLipo Triplex was a further devolopment in Laser Assissted Liposctuon which offers greater fat reduction and skin tightening and uses laser which are well absorbed by fat. The three wavelengths are combined in one laser output. Two wavelengths fire sequentially, liquefying the fat and tightening the skin with Sculptra and blood vessel coagulation.
The success we experienced with the original Smartlipo device which was invented by DEKA Laser Company in year 2000 after 12 years of extensive studies, was greater than we thought it would be.
Cosmetic and plastic Surgeons recognise patient’s demands for less invasive cosmetic procedures. It has been realised by everyone that having a clinic based only on plastic surgery is not enough, as these patients are going to clinics that offer Smartlipo treatments because Smartlipo provides good results and is less invasive than previous more invasive treatments.
Smartlipo is not a lunch break fat removal treatment and Dr Ayham Al-Ayoubi stresses that. Experienced techniques are required.

Dr Ayham Al-Ayoubi in year 2007 invented and developed the revolutionary technique of laser assisted liposuction and then when on to becoming the very first surgeon who pioneered this technique by using the very original SmartLipo machine known as the first generation Smartlipo DEKA system with suction of the liquefied fat. The Smartlipo DEKA system has a laser energy of 6 Watts, 10 Watts and goes up to 18 Watts
In the year 2008 the evolution of laser treatment came with the 2nd generation Smartlipo MPX which was also introduced by Dr Ayham Al-Ayoubi who furthermore evolved his laser assisted liposuction technique by using the Smartlipo MPX (which has 32 watts and 46 Watts laser energy) which allows him to perform Smart Lipo with suction in multiple areas in addition with the skin tightening effect.
The latest generation Smartlipo Triplex introduced by Dr Ayham Al-Ayoubi has taken the laser assisted liposuction technique to a different level this now allows the plastic surgeon to perform Smartlipo with suction on multiple and large areas giving skin tightening effect with no surgical scars and with no down time.
All the above Smartlipo treatments (with or without suction) can be performed under local anaesthetic or light IV sedation which in comparison with the traditional surgical techniques for fat reduction including liposuction makes the Smartlipo and the laser assisted liposuction treatments far more superior.
